Job Position :  Home Care Scheduler, Patient Care Scheduler, Patient Care Coordinator

Work Arrangement :  Fully Remote – Based in the Philippines or LATAM

Job Type :  Full-time, Independent Contractor

Work Schedule :   40 hours  per week |Shifts :   6am–2pm ET  or  2pm–10pm ET |  5 days per week (3 weekdays + 2 weekend days required)| Rotating schedule, with flexibility during peak demand periods and holidays.

About the Company

Our client is a U.S.-based tech-enabled scheduling service transforming the home care industry. Our AI-powered platform helps agencies ensure reliable caregiver coverage and deliver quality care to clients. They are growing quickly and looking for experienced  Remote Home Care Scheduler  to join their team and provide high-volume scheduling support for home care agencies.

This is not a typical call center role. Schedulers handle complex scheduling challenges, anticipate problems before they escalate, and partner closely with caregivers, clients, and agency staff to keep care delivery on track.

Role Overview :

As a  Home Care Scheduler  you will be part of our 24 / 7 scheduling coverage, managing inbound and outbound requests and ensuring seamless care coordination. You’ll work closely with our AI-enhanced platform, stepping in when human judgment and problem-solving are needed.

Key Responsibilities :

• Manage caregiver schedules, call-outs, and no-shows to ensure continuous client coverage
• Handle inbound (and some outbound) scheduling calls and texts from caregivers, clients, and agencies
• Use scheduling and EMR systems (WellSky, AxisCare, ClearCare, or similar) plus client's proprietary platform
• Proactively identify coverage gaps and resolve scheduling conflicts in real time
• Document updates accurately in the system to avoid missed calls or service disruptions
• Collaborate with teammates to ensure smooth handoffs across shifts and 24 / 7 coverage
• Provide feedback and suggestions to improve scheduling processes and the client's platform
